For patients who require admission to our hospital

一日でも早く日常の生活に 戻っていただきたいから、 万全のケア体制を整えています。
 

Our hospital is an acute hospital providing intensive care aimed at early discharge.
The mean duration of hospitalization is two weeks. From the viewpoint of the effective use of limited social resources, we ask patients to concentrate on treatment during hospitalization.
We consider that patients are our clients, and providing high-quality services to them is a very important aspect of treatment. For comfortable hospitalization, we assign several assistant nurses at each floor to provide daily care in addition to the highest nursing standard (seven nurses for one patient). As daily amenities are provided, you are able to manage sudden hospitalization.

Admittance to the hospital

Procedures for admittance

・Please fill out the required documents and bring them to the admissions desk on the 1st floor of the East Building.
・Please understand that the time of admittance may be delayed due to room preparation.
・If you are late for the appointed time of admittance, please inform the admissions desk on the 1st floor of the East Building.
・If there is any change in your health insurance card, please present it promptly to the ward clerk or admissions desk.
* Please note that if the presentation of your health insurance card is delayed, we may have to ask that you fully pay the expenses already occurred.

 

Things to bring

Please prepare the following daily items for admittance.

□ All medications you are taking currently, prescription record
□ Pajamas (open-front if possible), socks
□ Daily necessities
  Face towel, bath towel, tissue paper, usual shoes
□ Utensils necessary for meals
  Spoon, fork, straw, etc.
□ Toiletry
  Toothbrush, toothpaste, electric shaver, shampoo, rinse, body soap, etc.

Other items necessary for everyday life will be explained at the time of admittance.

Please use diapers and incontinence pads provided by our hospital.
Please write your name on the things that you bring.
Please manage your valuables by yourself.
A safety box is equipped for each bed, but please refrain from carrying a large amount of cash or valuables for the prevention of losses and theft.
Some electric appliances cannot be used.

Electric shaver, dryer, notebook PC, radio, mobile phone, electric blanket, etc.

○ Things that you cannot bring
TV, electric pot, refrigerator, things that use fire such as electric heaters, stoves, lighters, matches, etc., things with sharp edge such as fruit knives, scissors, shavers, etc.

* We will take your requirements into consideration, but please understand that since we give priority to patients who need private rooms for their treatment, we may be unable to meet your requirements.

* Charges for special rooms, private rooms, and two beds include charges for TV, Internet, and e-mail.

 

Meals

We provide meals according to the patient’s condition as directed by physicians.
If you have any food restrictions due to allergies, etc., please notify us in advance.
Meal times – Breakfast: 8:00 a.m. / Lunch: 12:15 / Dinner: 18:00 p.m.
Meals are provided at an appropriate temperature. You can choose rice or bread for breakfast.
A selective menu is provided for lunch every Wednesday and Thursday. We also provide an occasional menu according to each season.
A water heater is available at each ward.

 

Bath

You need a permission from your physician to take a bath.
Bath time: 9:00 a.m. to 15:00 p.m. (As bath time is different between each ward, your nurse will inform you.)

 

Outings/Overnight stays

If you wish to go out or stay overnight outside of the hospital, please get permission from your physician. Please ask your nurse.
Please fill out the form for permission.
Please return to the hospital at the time permitted. If you are late, please be sure to inform the hospital.

 

Telephone calls

Public phones are available on the 1st, 4th, and 5th floors of the East Building, and the 1st and 3rd floors of the West Building.
Please refrain from making phone calls at multiple bedrooms. Please use mobile phones at designated places (private rooms, staircase landings, access ways to the West and East Buildings) since the mobile phones may cause medical devices to malfunction.

 

Use of electric appliances

A TV, iPad, and refrigerator are available at each room.
(A total of 500 yen per day is required to use the TV and refrigerator.)
Please refrain from using the PC in such a way as to hamper treatment.

 

Cautions during admittance

Please be sure to keep yourself at rest required for treatment as directed by your physician.
We enforce a total ban on smoking throughout the premises of the hospital.
Please note that if you smoke in the hospital area (including electronic cigarettes), treatment may be discontinued or you may be forcibly discharged from the hospital.
If you are instructed by your physician to stop smoking (including electric cigarettes) for treatment reasons, treatment may be discontinued even if your smoking occurs outside of the hospital area.

Requests for patients

Explanation about medical conditions concerning treatment

We always make effort so that the patients requiring hospitalization can concentrate on treatment without anxiety. We set opportunities to explain worrisome medical conditions, treatment details, timing of discharge, etc. We ask for your kind cooperation in the following points.

 

Explanation of medical conditions

We will explain medical conditions during hospitalization as needed.
We will propose a date and time for explanation in advance. Please make an appointment on the proposed date and time as much as possible.
If your family member wishes to hear the explanation by your physician, please notify your nurse in advance. The nurse will inform you of the convenience of the physician on a later day.
Please understand that we cannot respond to telephone inquiries for the protection of personal information.

Discharge

If you are able to be discharged, your physician will explain the date and time of discharge. Discharge is usually made during the morning hours.
Please understand that we may not be able to meet your request for the date and time of discharge to ensure beds for emergency patients.

Visits

Visiting hours

Visiting hours: 13:00 p.m. to 20:00 p.m.
Visiting hours may be restricted according to the patient’s condition.
As we put the greatest priority on the patient’s condition, we may ask you to refrain from visiting according to the patient’s condition. If you wish to visit, please ask the staff station.

 

Persons who wish to visit

Please receive “Visitor Card” at the admittance desk on the 1st floor of the East Building.

 

Chaperone

If someone wishes to be a “chaperone,” the physician’s permission is required. Please ask the nurse for predetermined procedures.
We may ask for “chaperones” according to the patient’s condition.

Consultation by patients and families

Our social workers (counselors) will accept individual consultation from patients, including worries about continuing treatment. Please feel free to ask.
* Problems concerning life during hospitalization and after discharge, social rehabilitation, introduction of and procedures for various medical systems, etc.

Acceptance of apprentices

We accept doctors-in-training and student medical professionals (nurses, pharmacists, rehabilitation engineers, radiological technologists, etc.) as a training facility.
We give training and practice for medical care and nursing under the guidance of expert staff. We ask for your kind understanding and cooperation.

 

Questionnaire to measure patient satisfaction

We aim to provide medical care to satisfy all patients.
At the time of discharge, we will ask patients for honest opinions about our medical services.
(The chaperone can fill out the questionnaire.)
We ask for your kind cooperation to fill out the questionnaire form, which we will hand out before discharge.

 

Others

Please understand that we decline to receive any gifts, money, or goods for our staff.

Payment of hospital expenses

Hospital expenses are charged at the end of each month. After an invoice is issued, please make your payment at the accounting department on the 1st floor of the East Building. (Monday to Friday: 9:00 a.m. to 19:00 p.m. / Saturdays, Sundays, and holidays: 9:00 a.m. to 16:00 p.m.)
Payment can be made using various credit cards.
(Acceptable credit cards: VISA, JCB, American Express, MasterCard, MillionCard, UFJ Card, Nicos, UC, DC, MUFG CARD, Diners Club International, J-Debit, UnionPay
If you need an approximate estimate of expense, please ask the ward clerk.

(Beside the admission desk on the 1st floor of the East Building)

Monday to Friday: 7:00 a.m. to 19:00 p.m. / Saturday: 7:00 a.m. to 17:00 p.m. (only on the 1st and 3rd Saturdays)
Closed: Sunday and holidays

Convenience store

Laundry service 6th floor of the East Building (washing place, washing machine, dryer)
100 yen per 30 minutes
* Washing and drying in rooms are prohibited.
* Please prepare coins.

Laundry service

Cleaning (paid) 650 yen
We clean clothes, underwear, sleepwear, towels, etc. necessary for treatment.
It takes three days from collection to completion.
Tea service (free of charge) 1st, 3rd, 4th, and 5th floors of the East Building (The service is also available beside the elevator on the 1st floor of the East Building.)

Parking

There are six parking lots.
As the parking space is limited, we ask hospitalized patients to refrain from parking their cars. If you visit the hospital by car on the day of admission, please ask someone to accompany and drive.
